갑상선 기능을 지원하는 영양소들

 

요오드

신체는 갑상선 호르몬을 생산해내기 위해 요오드를 필요로 한다. 1990년대 후반에 32개의 유럽 국가들은 여전히 요오드 결핍의 영향을 받고 있었다. 2007년에 세계 보건 기구 (WHO)는 약 30% 이상의 세계 인구(20)가 요오드 불충분이며 소변의 요오드 수치가 리터당 100 마이크로그램 이하로 나타났다. 요오드 첨가 소금은 요오드 결핍을 예방하는데 효과적인 것으로 나타났다. Morton Salt Company 1924년에 미국에서 요오드 첨가 소금을 판매하기 시작했다.

 

태어나지 않은 아이의 갑상선 기능 부진증, 선천성 갑상선 기능 부진증 또는 크레틴 병은 요오드 결핍에 의해 자주 유발된다. 선진국가에서 이 질병들의 발생률은 약 4500분의 1이다. 하지만 크레틴 병의 발병률은 요오드 결핍의 지역에서 20분의 1 정도의 비율로 발생할 수 있다. 이 때문에 요오드 결핍은 정신지체의 가장 주된 원인으로 남아있다.

 

임신 중 T4의 생산량은 2배로 증가하며 하루 요오드 필요량이 더 많아진다. 요오드 결핍 임산부들은 자라나는 아이들의 정상적인 신경 발달에 필요한 갑상선 호르몬을 생산할 수 없게 되고, 학습 장애와 인지 능력에 장애가 있는 아이들을 낳을 확률이 매우 높다. 심지어 중등도의 요오드 결핍마저 태아의 IQ점수를 약 8~16점 감소시킨다.

 

요오드 첨가 소금을 피하는 사람들이나 소금을 섭취하지 않는 식습관을 가진 사람들은 요오드 결핍이 될 확률이 높다. 특히 요오드가 고갈된 토양에서 자라는 식물을 섭취하는 채식주의자들 또한 요오드 결핍이 될 가능성이 높다. 해조류를 피하는 엄격한 채식주의자들은 요오드 결핍의 위험이 더 높다.

 

너무 많은 요오드를 섭취하거나 너무 적은 요오드를 섭취하는 것은 모두 갑상선 기능 부진증과 연관이 되어 있다. 이것은 소변의 요오드 양이 높거나 낮은 사람들 모두에서 갑상선 기능 부진증이 연관되어 있다는 것으로 알 수 있었다. 높은 요오드 섭취량은 또한 하시모토 질환의 위험을 높인다.

셀렌

요오드 다음으로 셀렌은 아마 갑상선 기능에 영향을 주는 중요한 요소일 것이다. 갑상선은 다른 장기보다 더 많은 셀렌을 가지고 있다. 셀렌은 효소의 중요한 요소이며 T4로부터 요오드 분자를 제거하여 T3으로 전환시킨다; 셀렌 없이 갑상선 호르몬의 작용은 없을 것이다. 환자들이 다양한 형태의 갑상선 질병을 앓고 있을 시 셀렌 수치를 확인해보면 모두 정상적인 건강한 사람들 보다 더 낮은 수치를 나타냈다. 여러 연구원들은 셀렌 보충제가 T4 T3으로 전환되는 것을 도울 것이라고 말한다. 또한 셀렌은 갑상선 분비선 자체를 보호하는데에도 큰 역할을 한다. 갑상선의 세포들은 과산화수소를 생산하며 이것을 갑상선 호르몬을 만들어 내기 위해 사용한다. 셀렌은 이 반응으로 인해 발생하는 산화 손상으로부터 갑상선 분비선을 보호한다. 적당한 셀렌 없이 높은 양의 요오드 수치는 갑상선 분비선 세포들의 파괴로 이어진다.

2002년에 진행된 위약 조절된 연구의 독일 연구자들은 하시모토 질병을 앓고 있는 사람들과 높은 수치의 갑상선 페록시다아제 항체를 가지고 있는 환자들에게 매일 200 마이크로그램의 셀레늄을 제공하는 연구를 진행하였다. 3달 후 셀렌을 섭취한 환자들의 갑상선 페록시디아제 항체 수치는 처방전의 수치보다 66.4%가 줄어들었다. 환자들 중 9명의 항체 수치는 정상으로 돌아왔다. 호주 연구자들은 2008년에 대상자들을 높은 수치의 페록시디아제 항체를 가지는 인구로 제한을 하지 않았더니 이른 연구의 결과를 보일 수 없었다고 말했다. 그들은 셀렌 보충제가 더 높은 질병 활동을 가지는 환자들에게 더 이익이 될 것이라고 말한다.

아연

아연은 T3수치가 낮은 환자들에게 도움이 될 것이며, T4 T3으로 전환하는데 기여한다. 여러 동물연구에서 아연 결핍은 T3과 fT4 수치를 30%가량 낮추었다. T4의 수치는 아연 결핍에 의해 영향을 받지는 않았다. T4 수치는 일반적이지만 fT3 수치가 낮은 환자들의 집단에서 rT3 수치는 높고 경미하고 중도의 아연 결핍인 반면에, 12달 동안 경구 아연 보충제를 섭취하는 것은 혈청 내 fT3과 총 T3수치를 정상화시켰으며 rT3 수치를 감소시키고 TSH 수치를 정상화시켰다.

 

이와 반대로, 요오드와 같이, 너무 많은 아연은 갑상선 기능에 압박을 가할 수 있다. 매우 높은 양의 아연은 구리 흡수에 방해를 가하며 심각하고 치명적인 구리 결핍을 유발할 수 있다. 그러므로 아연을 보충할 때 구리를 함께 보충하는 것을 추천한다.

 

철 결핍은 갑상선 효소 페록시다아제의 활동을 줄이며 갑상선 호르몬의 생산을 방해한다. 한 연구에서 철결핍이 9.8%였던 제어집단에 비해 무증상 갑상선 부진증을 가지는 15.7%의 여성들이 철 결핍인 것으로 나타났다. 철 결핍으로 인한 빈혈증은 줄어들고 철 보충은 개선되며 요오드 보충제의 이익들을 볼 수 있다. 철 결핍 갑상선 부진증 환자들을 티록신(T4)과 철로 치료하는 것은 철 결핍으로 인한 빈혈증을 더 잘 개선할 것이다.

 

구리

2010 8월 연구에 따르면 구리는 정상적인 뇌 발달에 중요하며, 구리 결핍은 시상하부가 갑상선 호르몬을 적절히 조절하지 못하게 만든다. 구리가 결핍된 임신한 쥐들의 새끼들은 건강한 쥐에서 태어난 새끼들보다 T3 48% 더 적게 생산해낸다.

 

비타민 E

비타민 E는 갑상선 부진증으로 인해 발생하는 산화 스트레스를 감소시킬 수 있다. 한 동물연구에서 비타민 E는 갑상선 세포 손상과 산화작용으로부터 동물들을 보호하는 것으로 나타났다.

 

비타민 D

비타민 D의 결핍은 자가면역 갑상선 질병의 위험을 증가시킨다. 나이에 따라 조절되었을 때 642명의 집단(244 남성, 398 여성)에서 갑상선 항체의 존재는 비타민 D 수치와 역관계에 있는 것으로 나타났다. 더 나아가 다른 증거들에 따르면 비타민 D 결핍이 갑상선 암과 갑상선 혹을 가진 사람들에게 더 흔한 것으로 나타났다. 적당한 비타민 D는 많은 이익을 제공하기 때문에 필요하다면 보충하는 것이 좋다.

 

비타민 B12

일본인 연구자들은 DHEA, DHEA-황산염, 프레그네놀론 황산염 수치는 갑상선 기능 부진증의 환자들에게서 확실히 더 낮았다고 한다.

 

울금추출물

2002년 연구에서, 쥐를 사용한, 울금추출물을 사용한 치료가 화학적으로 유도된 갑상선 기능 부진증의 영향(갑상선의 무게, T3, T4, 콜레스테롤 수치)을 감소시켰다.

Nutrients to Support Thyroid Function

Iodine.

The body needs iodine to make thyroid hormone. As of the late 1990s, thirty-two European countries were still affected by iodine deficiency. In 2007 the WHO estimated that over 30% of the world’s population (2 billion people) has insufficient iodine intake as measured by urinary iodine excretion below 100 µg/L. Iodized salt has proven to be effective at preventing iodine deficiency. The Morton Salt Company began selling iodized salt in the United States in 1924.

 

Hypothyroidism in the unborn child, congenital hypothyroidism or cretinism, is frequently caused by iodine deficiency. In industrialized countries the incidence is about 1 case in 4500 live births. Yet, the incidence of cretinism can increase to as much as 1 case in 20 live births in areas that have iodine deficiency. Because of this, iodine deficiency remains one of the leading causes of mental retardation.

 

During pregnancy T4 production doubles, causing increases in daily iodine requirements. Iodine deficient pregnant women cannot produce the thyroid hormones that are needed for proper neurological development of their growing babies, and are at high risk of giving birth to infants with cognitive impairment and learning delay. Even moderate iodine deficiency in a pregnant woman can lower her infant’s IQ from 8 to 16 points.

 

People who avoid iodized salt or adhere to a salt-restricted diet may become iodine deficient. Vegetarians are also at risk of developing iodine deficiency, especially if they eat food grown in low iodine soil. Vegans that avoid sea vegetables, are also at higher risk.

 

Diets both low and high in iodine are associated with hypothyroidism. This is supported by studies that have shown that both low and high urinary iodine excretion are associated with hypothyroidism. High intake of iodine also increases the risk of Hashimoto’s thyroiditis.

 

Iodine or foods high in iodine, such as seaweed, are thought useful in treating hypothyroidism but this is probably only true for people who are iodine deficient. The upper intake level (UL) of iodine for adults is 1.1mg per day. However, iodine ingestion above this amount is generally well tolerated.

 

The amount of supplemental iodine needed for an individual varies widely based on the factors previously listed. It is important to test thyroid function when supplementing with iodine since both low and excessively high intake can contribute to thyroid dysfunction.

 

Selenium.

After iodine, selenium is probably the next most important mineral affecting thyroid function. The thyroid contains more selenium by weight than any other organ.118Selenium is a necessary component of the enzymes that remove iodine molecules from T4 converting it into T3; without selenium there would be no activation of thyroid hormone. When patients suffering from various forms of thyroid disease were tested for selenium levels, all were found to be lower than normal healthy people. Some researchers suggest that selenium supplementation will improve conversion of T4 to T3. Selenium also plays a role in protecting the thyroid gland itself. The cells of the thyroid generate hydrogen peroxide and use it to make thyroid hormone. Selenium protects the thyroid gland from the oxidative damage caused by these reactions. Without adequate selenium, high iodine levels lead to destruction of the thyroid gland cells.

 

People living in areas with low soil selenium content are more likely to develop Hashimoto's disease. This may be because a selenium deficiency makes the enzyme glutathione peroxidase less effective. Thus selenium supplementation has been suggested for treating Hashimoto’s disease.

 

In a placebo controlled study published in 2002, researchers in Germany reported on an experiment in which they gave 200 mcg of sodium selenite daily to patients with Hashimoto's disease and high levels of thyroid peroxidase antibodies. After three months, the thyroid peroxidase antibody levels of the patients taking selenium were decreased by 66.4% compared to their pre-treatment values, and antibody levels returned to normal in nine of the selenium treated patients. Austrian researchers reported in 2008 that they were unable to duplicate the results of the earlier study when they did not limit the study population to those with high levels of thyroid peroxidase antibodies. They suggest that selenium supplementation might be of greater benefit to patients with higher disease activity.

 

Selenium deficiency is also common in celiac disease, and this may be the tie-in to increased frequency of thyroid problems with celiac disease. During severe or prolonged infection, blood levels of selenium, T4, T3 and TSH decrease and the conversion of T4 to T3 slows, inducing a hypothyroid state. Because the enzymes that moderate this conversion require selenium, it has been hypothesized that supplementing extra selenium might prevent this decrease in T3 during illness. Supplying extra selenium may decrease mortality from infection, but it does not normalize thyroid hormone levels. It seems that the suppression of T3 during sickness is mediated by cytokines, in particular interleukin-6 (IL-6). It may be that IL-6 and other cytokines, generated by the infection, limit production of the selenium-enzymes and interfere with hormone production.

 

Zinc.

Zinc may be helpful in patients with low T3 and may contribute to conversion of T4 to T3. In animal studies, zinc deficiency lowered T3 and free T4 concentrations by approximately 30%. Levels of total T4 were not affected by zinc deficiency. In a group of patients with low levels of free T3 and normal T4, but elevated rT3 and mild to moderate Zn deficiency, taking oral zinc supplements for 12 months, normalized the serum free T3 and total T3 levels, decreased the rT3 and normalized TSH levels.

 

On the other hand, like iodine, too much zinc may suppress thyroid function. Very high doses of zinc interfere with copper absorption and can lead to serious and potentially fatal copper deficiency. Thus it is advised to take copper when supplementing with zinc.

 

Iron.

Iron deficiency hinders manufacture of thyroid hormone by reducing activity of the enzyme thyroid peroxidase. In one study 15.7% of women with subclinical hypothyroidism were iron deficient, compared to only 9.8 % of the control group. Iron-deficiency anemia decreases, and iron supplementation improves, the beneficial effects of iodine supplementation. Treating iron deficient hypothyroid patients with levothyroxine (T4) along with iron improves their iron deficiency anemia more than treatment with iron alone.

 

Copper.

An August 2010 study revealed that copper is important for normal brain development and its deficiency leaves the hypothalamus unable to regulate thyroid hormone effectively. Copper deficient pregnant rats give birth to infant rats that produce 48% less T3 than those born from healthy mothers.

 

Vitamin E.

Vitamin E may reduce the oxidative stress caused by hypothyroidism. In one animal study, vitamin E was shown to protect animals from increased oxidation and thyroid cell damage.

 

Vitamin D.

Deficiency of vitamin D may increase risk of autoimmune thyroid disease. When adjusted for age, presence of thyroid antibodies was inversely correlated with vitamin D levels in a group of 642 participants (244 males and 398 females) in New Delhi, India. Moreover, other evidence suggests that vitamin D deficiency is more common among individuals with thyroid cancer or thyroid nodules, compared to the general population. Given the many benefits of adequate vitamin D, it makes sense to supplement if needed.

 

Vitamin B12.

Hypothyroid patients are often vitamin B12 deficient. In a 2008 paper, Pakistani doctors reported that of 116 hypothyroid patients tested for vitamin B12, approximately 40% were deficient.146 It is not clear what the link between B12 deficiency and low thyroid function is, nor if thyroid function will improve with B12 supplementation. But, since low B12 causes serious neurologic damage, all hypothyroid patients should be tested.

 

DHEA and Pregnenolone.

Japanese researchers reported that concentrations of DHEA, DHEA-sulfate, and pregnenolone-sulfate are significantly lower in hypothyroid patients compared to age and sex matched healthy controls.

 

Turmeric (Curcuma longa) Extract.

A 2002 study, using rats, found that treatment with turmeric extract reduced the impact of chemically induced hypothyroidism in terms of thyroid weight, T4, T3 and cholesterol levels.

 

Rhodiola rosea.

Given the fact that stress can influence thyroid status, it may be beneficial for some individuals with hypothyroidism to consider adaptogenic herbs such as Rhodiola. Adaptogenic herbs support the adrenal glands and can improve the body’s response to stress.
