요오드는 정상적인 신진대사를 위해 반드시 필요하다. 우리는 식단을 통해 요오드를 얻는다. 갑상선은 요오드를 갑상선으로 집중시키는 요오드화나트륨 공동수송(NIS)이라는 작은 펌프를 가진다. 갑상선은 갑상선호르몬을 생산하기 위해 요오드를 필요로 한다. 갑상선호르몬은 건강한 신진대사를 위해 필수적이다. 따라서 우리는 식단에서 충분한 요오드를 섭취하지 않는다면 체중증가와 피로, 불임, 정신질환를 포함한 신진대사 저하를 겪을 수 있다.

 

흥미롭게도 유방에도 NIS 펌프가 있어서 모유수유시 요오드를 유방으로 집중시켜 아기들에게 요오드를 공급할 수 있도록 한다. 아기의 정신지체와 신체발달 지체를 막기 위해서 임산부는 임신 중에 충분한 요오드를 섭취해야한다. 세계의 몇몇 지역에서는 다른 지역에서 보다 요오드 결핍이 더 심하다.

심각한 요오드 결핍 식단을 섭취하는 사람들은 비정상적인 갑상선 호르몬 수치로 인하여 정신적/신체적 발달 지체인 크레틴병에 걸린 아이들을 가질 수 있다. 요오드 결핍은 갑상선과 유방, 식도, 난소, 자궁, 복부의 암 발생률을 증가시킬 수 있다. 또한 유방의 낭종과(섬유낭포성 유방질환) 난소의 낭종 형성을 유발하기도 한다.

 

미국과 다른 국가에서 요오드 결핍을 해결하기 위해 요오드를 식탁용 소금에 첨가한다. 하지만 몇몇의 사람들은 저염식 식단을 섭취하며 요오드첨가 소금을 사용하지 않는다. 더 나아가 염소(수영장 물, 스파 물, 수도), 불소(치약), 브롬(특정한 빵과 탄산음료)은 신체 내의 요오드의 자리를 대신하며 신체의 요오드 결핍을 유도한다. 미국의 대부분의 사람들은 최적의 요오드 결핍보다 낮은 상태이다.

 

결핍은 상대적인 단어이다. 얼마만큼 아픔을 느껴야 자기자신을 아프다고하는가? 만약 95%의 기력이 있다면 자기자신을 아프다고 할 것인가?  90% 80%? 심각한 요오드 결핍이 심각한 문제들을 일으킬 수 있으므로 가벼운 결핍이 가벼운 문제들을 일으키는 것은 말이 된다. 하지만 우리는 요오드 결핍과 같이 완벽하고 쉽게 피할 수 있는 것에서는 아무런 문제를 가지고 싶지 않다. 미국 추천 하루 권장량은 150mcg이다. 하지만 일본인들은 그것보다 50~80(하루 5~13mg)의 요오드를 대부분 해조류에서 섭취한다. 일본인들은 요오드 결핍과 관련된 건강 문제가 비교적으로 적다.

 

몇몇 사람들은 요오드가 부족하기 때문에 갑상선기능저하증을 앓고 다른 사람들은 적당한 요오드가 있음에도 갑상선에서 충분한 갑상선호르몬을 생산해내지 못한다. 둘의 차이를 어떻게 말할 수 있을까?

환자들이 갑상선기능저하증을 앓고 있으며(높은 TSH와 낮은 T3 T4) TPO 항체가 없다면 높은 TSH는 보통 요오드 결핍에 의한 것이다!(하시모토 병과 반대로)

 

소변 검사는 환자의 요오드 수치를 평가하기 위해 사용될 수 있다. 요오드 보충제는 충분한 요오드 양을 신장시키기 위해 사용될 수 있다. 사람들은 심계항진의 증상을 일으키는 열성 결절을 가진다면 많은 요오드를 섭취하면 안된다.

Low Iodine can cause weight gain, brain fog, fatigue, and infertility

Iodine is essential for the proper functioning of a normal metabolism.  We get iodine from our diet. The thyroid gland contains tiny pumps called Sodium Iodide Symporters (NIS) that concentrate the iodine in our thyroid glands.  The thyroid gland needs iodine to make thyroid hormones. As you know, thyroid hormones are necessary for a healthy metabolism.  Thus, if we don’t get enough iodine in our diets we can easily suffer the effects of a slower metabolism, which include weight gain, impaired mental function, fatigue, and infertility!

 

Interestingly, the breast also has the NIS pumps so that it can concentrate iodine in the breast in order to supply nursing babies with iodine.  It is extremely important that pregnant mothers have enough iodine in their systems during pregnancy in order to prevent stunted mental and physical growth of the baby.  Some areas of the world have diets more deficient in iodine than others.

 

People with diets severely deficient in iodine can have children with cretinism which is severe mental and physical growth retardation due to inadequate thyroid hormone levels. Iodine deficiency can increase cancer of the thyroid, breast, ovary, uterus, stomach, and esophagus.  It can also contribute to cysts in the breasts (as in fibrocystic breast disease) and ovaries!

 

The United States and many other countries add iodine to table salt to combat iodine deficiency.   However, some people are on low salt diets or choose to use non-iodized salt.

In addition, chlorine (pool water, city water, spa water), fluorine (from toothpastes), and bromine (from certain baked goods and certain soft drinks) can take the place of iodine in the body, leaving the body relatively deficient.  Most people in the United States have less than optimal iodine sufficiency.

 

Deficiency is a relative term.  How bad would you have to feel before you called yourself sick?  If you only had 95 % energy would you say you were sick?  What about 90%, or 80%.   Since we know that severe iodine deficiency can cause severe problems, it makes sense that mild deficiency can cause more mild problems. However, we don’t want to have any problems when it comes to something easily and totally avoidable like iodine deficiency.  The US recommended daily intake is 150 mcg/day.  However, Japanese people often consume 50-80 times that much (5-13 mg/day), primarily from seaweed, and have less of the health problems associated with iodine deficiency.

 

Some people have hypothyroidism because they are deficient in iodine, while others are hypothyroid because their thyroid glands can’t make enough thyroid hormone even though they have enough iodine on board.  How can you tell the difference?If patients have hypothyroidism (high TSH and low T3 and T4 ) and no TPO antibodies then the high TSH is usually due to iodine deficiency (as opposed to Hashimoto’s)!

 

Urine tests can be done to assess a patient’s iodine levels.  Iodine supplements can be used to promote optimal iodine sufficiency.  People should not take a lot of iodine if they have a hot nodule that is causing them symptoms of palpitations.
