하시모토 갑상선염 (Hashimoto’s thyroiditis)과 요오드, 셀레늄

Susan은 피로, 부종, 근력저하 등을 유발하는 자가면역 갑상선 질환인 하시모토병을 앓고 있었다. 지난해 동안 그녀는 씬지로이드라는 갑상선 호르몬제를 복용하며 갑상선 항체 수치를 측정해왔다. Susan은 그녀의 항-갑상선 항체 수치가 검사 때마다 계속 증가하는 것을 발견하고 걱정하고 있었다. 의사들은 정확한 원인을 발견하지 못했으며, 지푸라기라도 잡는 심정으로 나에게 찾아왔다.

 

셀레늄은 항체 수치를 낮출 수 있다

셀레늄은 필수 미량원소로 갑상선에 매우 중요하다. 셀레늄 결핍은 하시모토병의 원인에 관여하며, 셀레늄 보충이 하시모토 질환에 도움이 된다는 사실이 발견되었다.

 

갑상선 기능에 중요한 셀레늄

갑상선세포의 생리학에 관한 최근 보고는 셀레늄이 갑상선 기능에 매우 중요하다는 것을 보여준다. 체내에는 글루타치온 퍼옥시다제(glutathione peroxidase enzyme), iodothyronine deiodinases enzyme (티록신 (T4)를 활성형인 (T3)로 전환) 등을 포함한 적어도 30가지 이상의 셀레늄의존단백질(selenium dependent proteins)이 존재한다. 이러한 단백질들이 정상적으로 기능하기 위해서는 보조인자로서 셀레늄이 필요하다. 셀렌 단백질인 glutathione peroxidase는 갑상선 세포에서 생성된 과산화수소 (H2O2)에 의한 손상으로부터 갑상선 세포를 보호해준다. H2O2는 갑상선 호르몬 합성에 필요하지만, 너무 많은 양의 H2O2는 갑상선 세포를 손상시킬 수 있다. 셀레늄 결핍인 경우, 글루타치온 퍼옥시다제 (glutathione peroxidase enzyme)가 갑상선 세포를 보호할 수 없게 되어 과도한 H2O2로부터 세포가 손상을 입게 된다. 현재 이론은 이렇게 손상된 세포가 면역계에 외부물질로 인식되어 하시모토 자가면역 질환을 일으킨다는 것이다.

 

다시 환자(Susan) 얘기로 돌아와서

Susan에게 하루 400mcg의 셀레늄을 처방해주고, 갑상선약을 씬지로이드에서 훨씬 강력하고 우수한 임상적 효과를 나타내는 천연 갑상선 호르몬로 바꾸었다. 3개월 후, Susan의 항체수치는 유의하게 감소하였다.

 

갑상선 검사와 치료

올바른 갑상선 검사는 다음 lab values들을 포함한다: TSH, Free T3, Free T4, TPO Abs, Tgb Abs, reverse T3, 혈청 셀레늄 수치, 소변 요오드 수치. 갑상선 패넬(thyroid panel)은 임상병력과 검사에 따라서 부자되는 검사를 포함하는 대규모 검사의 일부분이다. 갑상선 치료에는 RLC labs에서 나온 Nature-thyroid와 같은 천연 갑상선 호르몬을 사용하는 것이 올바르다.  천연 갑상선 호르몬 양은 환자의 몸무게, 갑상선 lab values, 갑상선 기능에 따라 다르게 처방한다.

 

요오드 보충제 사용에 대한 반대 견해

“왜 아직도 갑상선 증상이 나타나는가?(2010)”의 저자인 Datis Kharrazian 박사, “갑상선기능저하증에 요오드 치료는 불에 기름을 붓는 것이다라는 제목의 글을 The Healthy Skeptic 블로그에 올린 Chris Kresser 등은 하시모토병 환자에 요오드를 사용하는 것에 대해 회의적이다. 그들은 요오드가 자가면역 갑상선 질환을 일으킨다는 Rose, Yoon, 그리고 다른 이들의 문헌들을 근거로 주장을 하였다.

 

요오드와 갑상선 과산화효소 (thyroid peroxidase, TPO)

Datis Kharrazian  Chris Kresser, 그리고 그들의 추종자와 제자들은 요오드가 갑상선 과산화효소 (thyroid peroxidase, TPO)를 자극한다고 한다. 이는 동물과 사람을 대상으로 TPO 효소 활성에 대한 요오드의 효과를 알아본 실험들과 대조적이다. 실험의 결과는 요오드가 TPO 효소의 활성을 억제한다고 한다.

 

의학 문헌에는 요오드 보충제가 자가면역성 갑상선염을 악화시킨다는 논문들로 가득 차 있다. 이들은 모두 TPO 티로글로불린 항체 수치 증가를 보여준다. 그리고 나를 포함한 많은 의사가 실제 임상 현장에서 이를 경험해왔다. 게다가, 나는 요오드를 복용한 환자에게서 “하시중독증(Hashitoxicosis)”라고 불리는 일시적 갑상선기능항진증(transient hyperthyroidism)이 일어난 사례를 보기도 했다.

 

하시모토병에 요오드를 사용하는 의사들

하지만 요오드 지지자인 Drs. Guy E. Abraham, David Brownstein, Jorge D. Flechas, David Derry는 하시모토병을 앓고 있는 많은 환자에게 요오드 치료를 성공적으로 사용해왔다. 야후 요오드 그룹 (3900명 회원 보유)와 같은 요오드 메시지 게시판에는 요오드 보충제를 문제없이 사용하여 회복하고 있다는 많은 하시모토병 환자의 글들이 올라와 있다.

 

누가 맞는 걸까?

 

셀레늄은 하시모토병 환자의 갑상선을 보호해준다

정답은 셀레늄에 있다. 셀레늄 결핍은 하시모토 갑상선염에서 요오드에 의한 갑상선 손상의 (근본적) 전제조건이 된다. 셀레늄 보충은 요오드에 의한 갑상선 손상을 보호하고 예방해준다. Dr K  Chris Kresser 같은 요오드 반대자와 이들이 인용한 문헌들은 셀레늄의 역할을 간과하고 있다.

 

Renato Iwakura

브라질공학자인 하시모토병환자 Mario Renato의 저널엔 하시모토에 대한 우수한 리뷰가 있다. 그는 다음과 같은 결론을 내렸다.

 

"문헌에 나온 조사 결과에 따르면 하시모토는 요오드 섭취에 영향을 받지 않는다. 이 문헌은 요오드 증가가 유해하고 요오드 제한이 하시모토 환자를 도울 수 있다는 세 가지 상황에 의해 왜곡되어있다.

1. 셀레늄 결핍인 경우 고용량의 요오드를 견디지 못하고 문제가 생긴다.

2. 해조류를 통해 요오드를 섭취하는 경우 해조류에 함유된 독성 금속과 할로겐도 섭취하게 된다.

3. 갑작스러운 요오드 증가는 reactive hypothyroidism을 유발할 수 있다.

 

요오드와 셀레늄을 같이 복용하고, 해조류 대신 요오드화칼륨을 섭취하고, 요오드 섭취량을 서서히 늘림으로써 위 세 가지 상황을 모두 피할 수 있다. "

 

Iwakura는 그의 결론을 지지해줄 Drs. Xu  Yang의 동물 실험을 인용하였다. 이 연구들에서 동물들()은 다양한 용량의 요오드와 다양한 양의 셀레늄을 섭취하였다. 2011 Xu박사는 그의 논문에 다음과 같은 결론을 내렸다.

 

"결론: 고용량의 요오드 섭취는 하시모토병의 모든 특성을 갖춘 자가면역 갑상선염을 일으킬 수 있다. 그러나 동물 실험에서 이러한 결과는 셀레늄이 결핍되거나 과잉상태일 때만 일어났다. 이와 유사하게 매우 높은 용량의 요오드 섭취는 셀레늄이 결핍되거나 과잉인 상태에서만 기존의 자가면역 갑상선질환을 악화시켰다. 적절한 셀레늄 수치에서 다양한 용량의 요오드를 섭취한 경우, 갑상선 여포세포는 건강하였으며 갑상선종이 사라지고 자가면역 표지자인 Th1/Th2 ratio  CD4+/CD8+ ratio 가 정상화되었다.

 

게다가 2007 Fan Yang European Journal of Endocrinology에 발표한 동물실험에서 그는 다음과 같은 결론을 내렸다:

 

"만성적인 고용량 요오드 섭취는 자가면역 갑상선기능항진증의 위험을 높이거나 무증상 갑상선기능항진증의 결과 및 발병률에 영향을 미치지 않는다. 이는 만성 고용량 요오드 섭취가 자가면역 갑상선기능항진증 발생원인의 환경적 요소가 아닐 수도 있음을 시사한다."

 

하시모토병에 셀레늄이 미치는 영향

Elias E. Mazokopakis 2007년 그리스 보고서에서 다음과 같이 언급한다.

 

HT (갑상선기능항진증)과 같은 AITD (자가면역 갑상선 질환) 환자에게 셀레늄 보충은 GPX TR활성을 강화하고 갑상선호르몬 합성 중 발생한 lipid hydroperoxides  H2O2의 독성농도를 줄임으로써 염증반응이나 면역반응에 영향을 미치는 것으로 보인다.

 

요오드, 차세대 비타민 D?

요오드는 필수영양소로, 요오드 결핍은 세계적인 보건문제이다. 일부 연구진들은 요오드 보충이 "차세대 비타민 D"가 될 것이라고 믿는다. 비타민 D도 처음에는 독성이 있다고 여겨졌지만, 이제는 알려진 것보다 더 많은 용량이 건강에 이롭다는 것이 받아들여졌다. 나는 이에 대해 동의한다. 요오드 결핍은 발달하는 아이들의 정신지체를 일으킬 뿐 아니라, 유산, 갑상선암, 유방암, 위암 등의 원인이 된다.

 

Zimmerman 박사는 2011년 그의 보고서에서 다음과 같은 결론을 내렸다. "주기적인 모니터링을 통해 요오드 결핍 집단을 예방한다는 것은 요오드결핍의 잠재적인 부작용을 줄여줄 비용대비 효과가 매우 좋은 방법이다.

 

셀레늄 보충은 항-갑상선항체 수치가 높거나 하시모토 갑상선염 환자들의 선행조건이다. 요오드 결핍은 건강상 위험이며 요오드 보충은 이롭다. 그러나 하시모토 환자에게 요오드를 처방하기 전에 셀레늄 보충이 이뤄져야 한다. 셀레늄은 저렴한 캡슐이나 태블릿 형태로 존재한다. 통상적인 selenomethionine은 일일 200-400mcg이다. 셀레늄은 고용량에서 독성이 있으므로 혈액수치 검사를 시행 후 관련 지식이 풍부한 의사의 감독하에 복용해야 한다.

 

나는 자가면역 갑상선염 (하시모토병) 환자에게 요오드 보충제를 처방하기 위해 앞서 언급한 프로토콜 즉 2-4주간 셀렌 보충(200-400mcg/day)을 먼저 시작하고, 그 이후 낮은 용량의 요오드 (225mcg/d)를 시작한다.

 

소변요오드 검사를 통해 요오드 수치를 검사하는 것이 중요하며, 관련 지식이 풍부한 의사의 감독하에 용량조절을 해가며 복용해야 한다.

Hashimotos Thyroiditis and Selenium

Susan has Hashimoto’s thyroiditis, an autoimmune thyroid disorder causing fatigue, puffy face and muscle weakness. For the past year, she had been under the care of an endocrinologist who started thyroid medication called Synthroid, and tested for thyroid antibody levels. Susan found it disturbing that her anti-thyroid antibody levels kept climbing higher on each subsequent lab test. The doctors had no explanation, so she asked me if there was something else that could be done.

 

Selenium Can Decrease Antibody Levels

Yes, selenium is an essential trace mineral important for the thyroid gland. Selenium deficiency has been implicated in the etiology of Hashimoto’s thyroiditis, and selenium supplementation has been found beneficial.

 

Why is Selenium So Important for Thyroid Function?

Recent advances in research into thyroid cell physiology shows that selenium is very important for thyroid function. There are at least 30 selenium dependent proteins, including the glutathione peroxidase enzyme, and the Iodothyronine deiodinases enzyme (this is the one that converts thyroxine (T4) to bioactive (T3). These proteins all need selenium as a co-factor in order to function properly. The selenoprotein, glutathione peroxidase, protects thyroid cells from damage by hydrogen peroxide (H2O2) produced by the thyroid cell. H2O2 is needed as a normal step in thyroid hormone production.  However, too much H202 can damage the thyroid cell. In the event of selenium deficiency, the glutathione peroxidase enzyme cannot do its job protecting the thyroid cell, and the thyroid cells are damaged by excess H2O2. The current theory is that this damaged cell material is then recognized by the immune system as foreign, leading to Hashimoto’s autoimmune disease.

 

Back to the Patient

Susan was started on selenium, 400 mcg/per day.  In addition, we switched her thyroid medication from Synthroid to natural dessicated thyroid, a more robust and clinically superior preparation. Three months later, Susan returned for follow up labs which showed a significant decline in antibody levels.

 

Thyroid Testing and Treatment

A good thyroid testing protocol includes the following lab values, TSH, Free T3, Free T4, TPO Abs, Tgb Abs, reverse T3, serum selenium level, and urinary iodine level. The thyroid panel is usually part of a larger evaluation with additional lab tests tailored to the clinical history and examination. A good thyroid treatment protocol uses natural dessicated thyroid medication such as Nature-throid from RLC labs. Dosage for natural dessicated thyroid medication varies, based on body weight, thyroid lab values and thyroid function.

 

Voices in Opposition to Iodine Supplementation in Hashimoto’s

Vocal opponents to the use of Iodine supplementation for Hashimoto’s patients include Datis Kharrazian (“Dr. K”), author of the 2010 book “Why Do I Still Have Thyroid Symptoms?”, and Chris Kresser of The Healthy Skeptic in his post “Iodine for hypothyroidism: like gasoline on a fire?”. (1-2). They quote the work of Rose, Yoon and and others who report that Iodine is a trigger for autoimmune thyroid disease. (4-7)

 

Iodine and the Thyroid Peroxidase Enzyme Activity

Students and followers of Datis Kharrazian (“Dr. K”) and Chris Kresser say that Iodine is a stimulator for the thyroid peroxidase enzyme (TPO). This is contrary to published animal and human studies which examine the effect of Iodine on TPO enzyme activity, and report Iodine inhibits the activity of the TPO enzyme.  .

 

Iodine Worsens Hashimoto’s Thyroiditis

There is No Doubt – However Selenium Protects

The medical literature is full of studies showing Iodine supplementation worsens autoimmune thyroiditis, with measurable increases in TPO and Thyroglobulin antibody levels.(4-7) And, many practitioners, including myself, have seen this in actual clinical practice. In addition, I have seen cases of transient hyperthyroidism, called “Hashitoxicosis” after iodine ingestion. There is little doubt about this.

 

Proponents of Iodine for Hashimoto’s

On the other hand, proponents of Iodine such as Drs. Guy E. Abraham, David Brownstein, Jorge D. Flechas and David Derry have successfully used Iodine treatment in many patients with Hashimoto’s. Iodine message boards, such as the Yahoo Iodine Group (with 3900 members) report many Hashimoto’s patients doing well and recovering on Iodine Supplementation.

 

Who is Right?

Who is right in this question?  How can these two diametrically opposed views be reconciled?

 

Selenium Protects the Thyroid Gland in Hashimoto’s

The answer is the role of selenium. Selenium deficiency is the underlying prerequisite for iodine induced thyroid damage in Hashimoto’s Thyroiditis. Selenium supplementation is protective and prevents thyroid damage from iodine. Iodine opponents such as Dr K and Chris Kresser as well as the studies they quote tend to ignore the role of selenium.

 

Renato Iwakura Reviews the Literature

An excellent review of Iodine and Hashimoto’s can be found in a two part article by Mario Renato Iwakura, a Brazilian engineer and Hashimoto’s thyroiditis patient who is intimately familiar with the hypothyroidism literature. (8-9) (see part one, see part two)

 

He concludes:

A survey of the literature suggests that Hashimoto’s is largely unaffected by iodine intake. However, the literature may be distorted by three circumstances under which iodine increases may harm, and iodine restriction help Hashimoto’s patients:

1. Selenium deficiency causes an intolerance of high iodine.

2. Iodine intake via seaweed is accompanied by thyrotoxic metals and halides.

3. Sudden increases in iodine can induce a reactive hypothyroidism.

 

All three of these negatives can be avoided by supplementing selenium along with iodine, using potassium iodide rather than seaweed as the source of iodine, and increasing iodine intake gradually.”  endquote Renato Iwakura.

 

Iwakura quotes animal studies which support his conclusion from Drs. Xu and Yang. (10-12) In these studies, animals (mice) were given varying amounts of iodine as well as varying amounts of selenium. Dr Xu concludes in his 2011 report (10):

 

Conclusion: Excess iodine intake can cause an autoimmune thyroiditis that bears all the characteristics of Hashimoto’s. However, in animal studies this occurs only if selenium is deficient or in excess. Similarly, in animal studies very high iodine intake can exacerbate a pre- existing autoimmune thyroiditis, but only if selenium is deficient or in excess. With optimal selenium status, thyroid follicles are healthy, goiter is eliminated, and autoimmune markers like Th1/Th2 ratio and CD4+/CD8+ ratio are normalized over a wide range of iodine intake. “(10)

In addition, human studies such as this 2007 report by Fan Yang in the European Journal of Endocrinology concluded:

Chronic iodine excess does not apparently increase the risk of autoimmune hyperthyroidism or influence the incidence and outcome of subclinical hyperthyroidism, which suggests that chronic excessive iodine intake may not be involved in the occurrence of autoimmune hyperthyroidism as an environmental factor. ” end quote Fan Yang

 

Role of Selenium in Hashimoto’s

Dr Elias E. Mazokopakis from Greece reports in 2007:

Selenium (Se) supplementation in patients with AITD (autoimmune thyroid disease), including HT (hyperthyroidism), seems to modify the inflammatory and immune responses, probably by enhancing plasma glutathione peroxidase (GPX) and thioredoxin reductase (TR) activity and by decreasing toxic concentrations of hydrogen peroxide (H2O2) and lipid hydroperoxides, resulting from thyroid hormone synthesis. (14)

 

Benefits of Iodine, Is Iodine the next Vitamin D ?

Iodine is an essential nutrient, and globally, Iodine deficiency is a massive health problem. Some researchers believe that Iodine supplementation will become the “next Vitamin D”, an example of a vitamin which was initially thought to be toxic, and now is accepted in much higher doses as beneficial for health. (15-16) I tend to agree with this conclusion.  Not only is iodine deficiency a leading cause of mental retardation in developing children, and miscarriage in mothers, it is also a cause of thyroid cancer, breast cancer and gastric cancer in adults.(16) (17)

 

Dr Zimmerman concludes in his 2011 report (16).  He says:

Iodine prophylaxis of deficient populations with periodic monitoring is an extremely cost effective approach to reduce the substantial adverse effects of iodine deficiency throughout the life cycle. (16)”

 

Selenium supplementation is a prerequisite in all patients with elevated anti-thyroid antibody levels and Hashimoto’s thyroiditis. Iodine deficiency is a health risk and Iodine supplementation is beneficial. However, Selenium supplementation is required before giving Iodine to the Hashimoto’s patient.  Selenium is inexpensive and readily available as a supplement in tablet or capsule form. The usual dosage is 200-400 mcg/day of seleno-methionine. Selenium can be toxic at excessive dosage, so it is best to measure selenium blood levels, and work closely with a knowledgeable physician. For Iodine supplementation in autoimmune thyroiditis (Hashimotos) patients we follow a protocol described here, which starts off with selenium supplementation (200-400 mcg/d) for 2-4 weeks. After which, low dose (225mcg/d) iodine supplementation may be started. Again it is best to monitor iodine levels with spot urine iodine testing, and work with a knowledgeable physician for starting dosage.
