세포사멸과 암
요오드는
세포사멸 (apoptosis)를 유발한다. 프로그램 세포사는
성장 및 발달(손가락 사이 조직의 세포사멸을 통해 태아의 손가락이 형성)에 필수적이며, 암세포나 바이러스에 감염된 세포와 같이 조직에 위협을
주는 세포들을 파괴시키는데 필요하다.
Zhang과 그의 동료들은 요오드 흡수와 단백질 요오드화를 도와주는 유전자들 (Na+/I- symporter와 thyroperoxidase 유전자)을 사람 폐암세포에 스플라이스(splice : 유전자나 DNA 등을 접합) 하였다. 무기
비방사선 요오드는 in vitro에서 이러한 유전자 변형 암세포
95%이상의 세포사멸을 유도하였다. 이를 쥐에 이식할 때 요오드는 일반종양에 비해 유전자
변형된 이식종양 성장을 효과적으로 제한하였다.
Sekiya는 해조류의 한 종류인
Mekabu가 배양된 사람 유방암 세포주에 주는 효과에 대해 연구하였다. 그들은 Mekabu가 caspase 3, 6, 8의 활성화를 통해 세포사멸을
유도함을 제시하였다. Shrivastava와 그의 동료들은 최근 분자요오드도 사람 유방암 세포주에서 caspase 비의존성, 미토콘드리아 경유 세포사멸을 유도할 수 있음을
제시하였다. 역학적 연구들은 고용량의 요오드 섭취가 유방암 발병률 감소와 상관관계가 있음을 제시한다. 동물실험은 요오드가 유방암 발병률을 낮춰준다는 증거를 제시하며, 이러한
역학적 발견들의 인과적 연관성을 주장한다. Garcia-Solis와 그의 동료들은 발암물질 nitrosomethylurea를 주입한 쥐에서 유방암 예방에 요오드가 미치는 영향을 분석하였다. 분자요오드는 암 발병률이 73%인 대조군에 비해 발병률을 30%까지 유의하게 낮추었다. 이는 요오드화칼륨(potassium iodide)보다 분자요오드에서 더 효과적으로 나타났다. 발암물질로 DMBA를 사용한 또 다른 실험에서도 동일한 효과를 보였다.
암세포가
될 확률이 가장 높은 유방의 ductal cells에서 갑상선의 것과 동일한 요오드 펌프인 NIS 발견되었다. 이는 요오드가 유방암을 예방한다는 가설에 대한
생물학적 타당성에 힘을 실어준 또 다른 증거이다. 또한 요오드 결핍으로 인한 갑상선종 환자들의 유방암
발병률이 3배 더 높은 것으로 나타났다.
무기 요오드의 기타 기능들
아직 추가적인
연구가 필요하지만 요오드는 갑상선 외 다른 장기에서도 다음에 나열된 여러 다른 역할들을 해낸다. 요오드는
독성화학물질 (toxic chemicals)과 생물학적 독소
(biological toxins)를 제거한다; 자가면역을 억제한다; T세포 적응면역시스템(adaptive immune system)을
강화시킨다; 위에 존재하는 박테리아 특히 Helicobacter
pylori의 비정상적인 증식으로부터 보호한다.
요오드 농축이 가능한 장기들
갑상선
외에 다른 장기들도 NIS 펌프를 지니고 있다. 방사성 요오드
투여 후 실행한 total-body scintiscans은 정맥 주사한 요오드가 위 점막에 흡수됨을
보여준다. 수유 중인 유선과 타액선은 갑상선과 거의 대등한 양의(혈청
요오드치의 40배 이상) 요오드를 농축시킨다. T세포 적응면역계의 본부인 흉선은 무기 요오드를 흡수한다.
Scintiscans는 뇌척수액을 만드는 맥락얼기와 피부, 관절, 동맥, 뼈에서의 요오드 흡수 증가도 보여준다.
전문가의 의견
이러한
자료들은 갑상선이 요오드를 필요로 하는 유일한 장기라는 용인된 견해에 대한 의문을 제시한다. 진실에
대한 새로운 길을 찾을 때는 반드시 전문가의 의견에 의해 가로막힐 것을 예상해야 한다. 다음의 발언은
일반적으로 인정되는 견해에 인용되는데 특히 요오드일 때 해당된다. 요오드에 대한 전문가의 의견이라 함은
현재 갑상선 전문가들의 견해로 그들은 요오드의 갑상선외 작용을 무시하곤 한다. 많은 갑상선 전문가들은 1~2 mg 이상의 요오드 섭취가 Wolff-Chaikoff effect와 TSH 농도 증가를 유도함을 거론하며 잠재적으로 유해하다 주장한다.
Wolff-Chaikoff 효과는 요오드 섭취증가로 인한 일시적인 갑상선 호르몬 합성 억제 현상으로 임상적 의의는 없다. 증가된 TSH는 아무런 갑상선기능저하증의 증상이나 징후를 동반하지
않는다. 일부에서 mg의 요오드, 일반적으로 하루 50mg 이상을 섭취 후 갑상선기능저하증의 증상
없이 갑상선이 약간 부어오르기만 하는 경우도 때때로 관찰된다. 갑상선 질환이 없는 대부분의 사람들은
갑상선 기능에 아무런 임상적 이상반응 없이 하루 10~200mg의 요오드를 복용할 수 있다. 명백한 사실은 만들어진 지 26년이나 된 요오드의 RDI는
너무 낮다는 것이다.
요오드의 치료효과
요오드는
처음 갑상선종을 치료하기 위해 사용되었다. 갑상선종을 치료하는데 가장 효과적이었던, 오랜 기간 비밀로 지켜져 온 Coventry Remedy가 1977년 공개되었다. 이는 태운 바다해면인 것으로 밝혀졌다. 1811년 Bernard Courtois는 화약을 만들기 위해 soda를 끓여 질산칼륨(saltpeter)을 제조 중 요오드를 발견하였다. Andrew Fyfe는 1816년
sea sponge가 고용양의 요오드를 함유하고 있음을 발견하였다. 이러한 발견들에 뒤이어 French physician, Jean Coindet는 요오드팅크(iodine
tincture)가 갑상선종의 크기를 줄임을 보고하였다. 요오드를 사용한 갑상선종 예방에
관한 첫 번째 대조군 연구는 85년 전에 오하이오주 Akron의
학생들을 대상으로 진행되었다. 치료그룹은 하루 평균 11mg의
요오드화나트륨(sodium iodide)를 받았으며 대조군은 그러지 않았다. 2.5년 후 요오드 치료군의 갑상선종 발병률은 0.2%인 반면 대조군은 22%로 110배 차이를 나타내었다.
19세기와 20세기 초 요오드는 다양한 질병 특히 매독과 만성폐질환에
사용되어왔다. 수년 동안 의료진들은 기관지천식(bronchial
asthma)와 만성 폐쇄성 폐질환(chronic obstructive pulmonary
disease)을 치료하기 위해 환자에게 요오드화칼륨(potassium iodide)를
투여하였다. 1.5~3g부터 시작하고 10g까지 천천히 증량한
몇 주간의 치료는 놀랍도록 적은 부작용과 좋은 결과를 낳았다. 비타민
C를 발견한 노벨수상자 Albert Szent Györgi (1893-1986)는 "내가
의대생일 때 KI형태의 요오드는 보편적으로 사용되는 약이었다. 어느
누구도 그것이 정확히 어떤 작용을 하는지는 몰랐지만 무언가 좋은 결과를 만들었다"고 기술하였다. 이 당시 표준용량은 1g으로
770mg의 요오드를 함유한다.
피부
요오드는
피부과전문의들에 의해 21세기까지 g 용량으로 사용되어왔다. 그들은 nodular vasculitis와 pyoderma gangrenosum 등의 염증 피부염(inflammatory
dermatoses)을 SSKI (supersaturated potassium iodide)로

Apoptosis and Cancer
Iodine induces apoptosis. This process of
programmed cell death is essential to growth and development (fingers form in
the fetus by apoptosis of the tissue between them) and for destroying cells
that represent a threat to the integrity of the organism, like cancer cells and
cells infected with viruses. It is hypothesized that anticancer function may
well prove to be iodine’s most important extrathyroidal benefit. Zhang and
coworkers spliced genes that enhance iodine uptake and iodination of
proteins.the iodine pump (sodium-iodide symporter) and thyroperoxidase
genes.into human lung cancer cells. Inorganic, nonradioactive iodine induced
apoptosis in more than 95 percent of these genetically modified cancer cells in
vitro. When implanted in mice, iodine effectively limited the growth of the
genetically modified tumor xenografts, compared with unmodified tumors (p <
0.001). Sekiya et al. have studied the effect of Mekabu, a kind of seaweed, on
a cultured human breast cancer cell line. They have shown that it induces
apoptosis by activating caspases 3, 6, and 8. Shrivastava and coworkers have
recently shown how molecular iodine (I ) can also induce apoptosis in human
breast cancer cell lines using a caspase-independent and mitochondrial-mediated
apoptotic pathway. Epidemiological studies show that a high intake of iodine is
associated with a low incidence of breast cancer, and a low intake with a high
incidence of breast cancer. Animal studies provide evidence that iodine reduces
the incidence of breast cancer, arguing for a causal association to these
epidemiological findings. Garcia-Solis and coworkers analyzed the effect of
iodine on preventing breast cancer in rats given the carcinogen
nitrosomethylurea. Molecular iodine (I ) was more effective than potassium
iodide, ignificantly reducing the cancer incidence to 30 percent, compared to
73 percent in controls ( < 0.05). Another study, using DMBA as the
carcinogen, showed the same effect (p<0.01).
Other evidence, adding biologic
plausibility to the hypothesis that iodine prevents breast cancer, includes the
finding that the ductal cells in the breast, the ones most likely to become
cancerous, are equipped with an iodine pump, the sodium-iodide symporter, the
same one the thyroid gland has, to absorb this element. And people with goiters
resulting from iodine deficiency have a three times greater incidence of breast
cancer.
Other Functions of Inorganic Iodine
Extrathyroidal
iodine also has other functions, which warrant further study. It removes toxic
chemicals and biological toxins; suppresses autoimmunity; strengthens the
T-cell adaptive immune system; and protects against abnormal growth of bacteria
in the stomach, Helicobacter pylori in particular.
Organs with Iodine-Concentrating Ability
In addition to the thyroid, other organs
have a sodium-iodide symporter pump. Total-body scintiscans taken after
administration of radioactive iodine ( I) show that stomach mucosa absorbs
intravenously injected iodine. Lactating mammary glands and the three paired
salivary glands concentrate iodine almost to the same degree as does the
thyroid, in a 40-fold increase over serum levels. The thymus, seat of the
T-cell adaptive immune system, absorbs inorganic iodine. Scintiscans also show
an increased uptake of iodine by the choroid plexus, which makes cerebrospinal
fluid; in skin; and by joints, arteries, and bone.
Expert Opinion. and Iodine
These data call into question the accepted
view that the thyroid gland is the only organ that needs iodine. The following
statement applies to .accepted views. in general and to those about iodine in
particular: .When you seek a new path to truth, you must expect to find it
blocked by expert opinion.. Expert opinion on iodine is now the purview of
thyroidologists, and they often discount or ignore extrathyroidal actions of
iodine. Many thyroidologists consider an iodine intake greater than 1 to 2 mg
to be potentially harmful, citing theWolff-Chaikoff effect and an increase in
TSH (thyroid stimulating hormone) concentration with intakes greater than this
amount. The Wolff-Chaikoff effect, a temporary inhibition of thyroid hormone
synthesis that supposedly occurs with increased iodine intake, is of no
clinical significance. And an elevated TSH, when present, is .subclinical;
i.e., it is not accompanied by any signs or symptoms of hypothyroidism. Some
people taking milligram doses of iodine, usually more than 50 mg/day, develop
mild swelling of the thyroid without symptoms of hypothyroidism. The vast
majority of people without thyroid disease can take iodine in doses ranging
from 10 mg/day to 200 mg/day without any clinically adverse affects on thyroid
function. The truth is that the 26-year-old RDI for iodine is far too low perhaps, citing the Japanese example,
two orders of magnitude too low.
Therapeutic Use of Iodine in the Past
Iodine was first used to treat goiters. In
1779 the Coventry Remedy for goiter, which was very effective in treating this
disease and long held secret, was published. The remedy turned out to be burnt
sea sponge. Then in 1811 Bernard Courtois, brewing soda to make saltpeter for
gunpowder, discovered iodine. Andrew Fyfe, in 1816, found that sea sponge
contains high quantities of iodine. Following this chain of evidence, a French
physician, Jean Coindet, showed that tincture of iodine shrinks goiters. The
first controlled study using iodine to prevent goiters was done 85 years ago in
school-aged children in Akron, Ohio. The treatment group received an average of
11 mg/day of sodium iodide and the control group nothing. The iodine treatment
group, 2.5 years later, had a 0.2 percent incidence of goiter, whereas the
control group had a 22 percent incidence of goiter, a 110-fold difference.
Iodine was used for a wide variety of ailments in the 19 and early part of the
20 centuries, especially for syphilis and chronic lung disease. For many years
physicians used potassium iodide in doses starting at 1.5 to 3 g increasing up
to more than 10 g/day for weeks at a time to treat bronchial asthma and chronic
obstructive pulmonary disease, reportedly with good results and surprisingly
few side effects. The Nobel laureate Albert Szent Györgi (1893.1986), the
physician who discovered vitamin C, writes: .When I was a medical student,
iodine in the form of KI [potassium iodide] was the universal medicine. Nobody
knew what it did, but it did something and did something good.. The standard
dose was 1 g, which contains 770mgof iodine.
Iodine in Dermatology
This 19 century medicine continues to be
used in gram amounts in the 21 century, by dermatologists. They treat
inflammatory dermatoses, like nodular vasculitis and pyoderma gangrenosum, with
SSKI (supersaturated potassium iodide), beginning with an iodine dose of 900
mg/day, followed by weekly increases of up to 6 g/day as tolerated. Fungal
eruptions, like sporotrichosis, are treated initially in gram amounts with
great effect.
