갑상선 질환은 세계적으로 상당히 흔하며, 2억명 이상의 사람들이 갑상선 질환을 가지고 있다. 갑상선은 목 앞에 있는 나비모양의 샘으로, 체내 다양한 장기들의 적절한 기능을 위해 필요한 중요한 호르몬을 생산한다. 갑상선은 신체의 신진대사율 뿐만 아니라 심장 및 소화기능과 뇌 발달, 근육조절, 뼈 유지를 조절한다. 갑상선 질환은 갑상선의 구조 또는 기능에 영향을 주는 질환으로, 적절한 신진대사를 위해 필요한 호르몬들을 생산하는 능력에 영향을 미친다. 갑상선 세포의 성장 질환은 양성 또는 악성이 될 수 있는 결절 형성을 일으킬 수 있다.

 

다양한 증상들을 유발하는 많은 종류의 갑상선 질환이 있다. NCBI(National Center for Biotechnology Information)에 따르면 약 4200만명의 인도 사람들이 갑상선 관련 질환을 가지고 있으며, 이들 중 60%가 여성이다. 갑상선 기능 이상을 가진 인도여성 8명 중 7명이 젊은 여성이었으며, 이는 남성과 비교했을 때 여성에서 더 많이 관찰되었다.

 

남성보다 여성에서 갑상선 영향을 더 많이 받는 이유

갑상선 질환의 대부분은 자연적으로 발생된 자가면역질환이다. Dr Sunil Kumar Mishra는 이 질환이 환자의 면역시스템을 공격하고, 갑상선이 파괴되거나 자극을 받는 상태라고 말한다. Medicity 또한 우리에게 남성보다 여성에서 갑상선 질환이 더 흔한 이유를 말해주었다. 여러 연구에 따르면, 셀리악병, 1형 당뇨병, 염증성 장 질환, 다양한 경화증, 류마티스 관절염과 같은 자가면역 질환은 여성에서 더 흔하다. 감지하기 어려운 여러 증상 때문에 이러한 질환의 진단과 치료가 지연될 수 있다. 자가면역 질환을 제외하고 요오드 결핍, 특히 요오드를 더 많이 필요로 하는 임신 중에 요오드 결핍은 여성에서 더 많은 갑상선 질환을 일으킬 수 있다고 Dr Mishra는 덧붙였다. 요오드 결핍은 다양한 질환을 유발하는 갑상선호르몬 수치 감소를 일으킬 수 있다.

 

흔한 갑상선 질환의 종류

갑상선기능저하증, 갑상선기능항진증, 갑상선종, 갑상선염, 갑상선암, 갑상선 결절은 남성과 여성 모두에서 흔한 갑상선 질환이다. 이들 중 갑상선기능저하증과 갑상선기능항진증은 남성보다 여성에서 10배 더 흔하다.

 

갑상선기능저하증

갑상선기능저하증은 갑상선이 정상보다 덜 활성화되고 더 적은 호르몬을 생성할 때 유발되는 갑상선 질환이다. 갑상선기능저하증은 체내 호르몬 및 대사 균형을 방해한다. 여성에서 갑상선기능저하증의 가장 흔한 원인 중 하나는 하시모토병이라 불리는 자가면역질환이다. 하시모토병은 항체가 점차적으로 갑상선을 표적으로 하여 갑상선호르몬을 생성하는 능력을 파괴한다. 11명 중 10명의 여성이 살면서 갑상선기능저하증이 발달되는 것으로 추정된다.

 

갑상선기능항진증

갑상선기능항진증은 갑상선이 과다하게 활성화되고 정상보다 더 많은 호르몬을 생성하는 갑상선 질환이다. 이 호르몬들은 몸의 신진대사에서 중요한 역할을 한다. 갑상선기능항진증에서 갑상선은 비대해진다. 이는 몸의 신진대사를 증가시키고 갑작스런 체중감소와 초조함, 빈맥 또는 불규칙한 심장박동을 일으킨다.

 

갑상선 질환의 초기 경고증상

초기에 갑상선 질환은 애매한 증상으로 인해 종종 발견하지 못한다. 갑상선 질환은 불임과 지질 장애, 빈혈 또는 우울증을 유발할 수 있다. 이 증상들은 말기 상태에서 발생한다.

New Delhi: Thyroid disorders are quite common worldwide, affecting over 200 million people across the globe. The thyroid gland is the butterfly-shaped gland in front of the neck which produces hormones that are important for adequate functioning of the various organs in the body. They regulate the body’s metabolic rate as well as cardiac and digestive functions, brain development, muscle control and bone maintenance. Thyroid disorder is any condition that affects the structure or functioning of the thyroid gland, impacting its ability to produce the hormones required for proper metabolism. Disordered growth of thyroid cells can lead to the formation of nodules which can be benign or malignant.


There are many types of thyroid disease that can cause a variety of symptoms. According to NCBI (National Center for Biotechnology Information), about 42 million people in India suffer from thyroid-related disorders, of which 60% are women. One out of every eight young women in India have thyroid dysfunction and it is observed to be higher in women as compared to men.


Why thyroid affects women more than men

Most of the thyroid disorders are autoimmune in nature i.e. a condition in which a patient’s immune system attacks and either destroys or stimulates the thyroid gland, said Dr Sunil Kumar Mishra, Director, Endocrinology and Diabetology, Medanta - The Medicity, who also tells us why women are more likely to have thyroid than men. According to various studies, auto-immune diseases such as celiac disease, diabetes mellitus type 1, inflammatory bowel disease, multiple sclerosis and rheumatoid arthritis are more common in women. The diagnosis and treatment of these diseases get delayed because of varied symptoms which are difficult to detect. Apart from auto-immune diseases, iodine deficiency, especially during pregnancy when the body needs more iodine, can result in more thyroid disorders among women, Dr Mishra added. Iodine deficiency can lead to a drop in the thyroid hormone levels causing various disorders.


Common types of thyroid disorders

Hypothyroidism, hyperthyroidism, goiter, thyroiditis, thyroid cancer and thyroid nodules are common thyroid disorders affecting both men and women. Of these, hypothyroidism and hyperthyroidism are 10 times more common in women than in men.


Hypothyroidism is a type of thyroid disorder which is caused when the thyroid gland becomes less active and produces fewer hormones than normal. It disturbs the balance of hormones and metabolism in the body. One of the most common causes of hypothyroidism in women is the autoimmune disease called Hashimoto’s disease, in which antibodies gradually target the thyroid and destroy its ability to produce thyroid hormone. It is estimated that one out of 11 women will become hypothyroid in their lifetime.


Hyperthyroidism is a thyroid disorder in which a person’s thyroid gland becomes overactive and produces more hormones than normal. These hormones play an important role in controlling the body’s metabolism. In hyperthyroidism, the thyroid gland becomes enlarged. This increases the body’s metabolism and results in sudden weight loss, fast or irregular heartbeat, and anxiety.


Early warning signs and symptoms of thyroid disorders

Thyroid disorders are often missed in its early stage due to ambiguous signs and symptoms. It can be confused for infertility, lipid disorder, anaemia or depression. The symptoms occur only in the late decompensated state.


Symptoms for hypothyroidism may include:


  • Fatigue
  • Dry skin
  • Muscle cramps
  • Constipation
  • Intolerance to cold
  • Puffy eyelids
  • Excessive weight gain
  • Irregular menstrual flow

Symptoms for hyperthyroidism may include:


  • Nervousness
  • Difficulty sleeping
  • Weight loss
  • Moist palms
  • Rapid and irregular heartbeat
  • Bulging eyes
  • Unblinking stare
  • Vision changes
  • Excessive hunger
  • Frequent bowel movements

Risk factors for thyroid disorders

Anyone can develop a thyroid disorder, however, certain factors can increase a person’s risk of developing the condition. These include:


  • Family history of thyroid disease
  • Co-existing auto-immune condition
  • History of neck radiation
  • Thyroid surgery
  • Recent history of thyroiditis (inflammation of the thyroid)
  • Low iodine intake

Prevention

There isn’t any specific step you can take to definitively prevent a thyroid disorder. But as with other conditions, adopting a healthy lifestyle - such as eating a nutritious diet that includes iodine-rich foods, exercising regularly, keeping body weight in check, managing stress and not smoking - can help manage your condition or reduce the risk.


Timely diagnosis and proper treatment of thyroid disorder is essential in preventing an increase in the severity of this condition. If one experiences symptoms that hint a thyroid-related disorder, various blood tests may be conducted to evaluate the level of thyroid hormones in the body. It is advised that women should undergo an annual screening of thyroid gland for earlier detection of the problem.
